Brookline Dermatology Patient Forms
To schedule an appointment in our Brookline office, please call 617-277-0800, and for appointments in our West Roxbury office, call 617-608-1575. We participate in Medicare as well as most managed care plans.
Co-pays, Non-covered Procedures and Cosmetic Procedures:
All co-payments must be paid at the time of service. This arrangement is part of your contract with your insurance company. Co-payment and co-insurance are determined by your insurance company. Full payment for cosmetic procedures is due at the time of service. We accept cash, check, Mastercard, Discover and Visa.
If your insurance company requires a referral from your Primary Care Physician, it is your responsibility to obtain one. If a valid referral is not on file, you will be asked to sign a referral waiver on the appointment day. If the referral is not sent to us within 10 days of the appointment having taken place, you will be responsible for the cost of services rendered.
We participate in most insurance plans, including Medicare. We will submit your claims and assist you in any way we reasonably can to help get your claims paid by insurances we are contracted with. It is your responsibility to know your benefits. Please contact your insurance company directly with any questions you may have regarding your coverage.
An annual deductible is the dollar amount you must pay out of pocket during the year, as dictated by your insurance company. When that figure is met, your insurance will then begin to pay.
Credit Card on File:
As a guarantee of payment Brookline Dermatology is now keeping credit cards on file. We use a secure encrypted gateway that is completely compliant as required by law. Credit cards will only be charged after the visit has been billed to insurance, and a balance is indicated. Before we ever charge your credit card, we will send two monthly statements allowing you the opportunity to question a bill if need, and to pay conveniently through the mail. Your card will only be charged if there is no response to our bill. Please call if you have any questions.
How will I know how much you are going to charge me?:
You will receive an Explanation of Benefits in the mail from your insurance carrier that explains how much of your office visit or service they pay and how much you pay. This explanation tells you exactly, according to your coverage plan, how much of your health care bill is your responsibility.
What if I do not have a credit card?:
You are welcome to leave a debit card, Health Saving Account or Flex Plan Card on file with us.
We make every effort not to overbook our patients. No shows prevent other patients from getting timely appointments. Please give us 24 hours notice if you must cancel your appointment. This will allow time to schedule another patient. It is our policy to charge a fee for missed appointments or for those cancelled with less than 24 hours notice. We are here to make your visit a pleasant one!
